By signing below, I confirm that I am the registered attendee listed below, that I am attending today's Camera Basics for Cavemen workshop voluntarily, and that I have read, understand, and agree to the Camera Basics for Cavemen Workshop Participation Agreement.

I understand that today's workshop may be photographed, filmed, and/or audio recorded by Primitive Recordings LLC, Primitive Pictures, and/or Next Door Vegas. I understand that if I do not want to appear in close-up or featured promotional media, I must notify staff before the workshop begins.

I also agree to follow all instructor and staff directions, avoid touching equipment unless invited, and respect the venue, staff, instructors, and other attendees.
